UPDATED 1/20/11

Filling out forms ranks right up there with going to the dentist for me. (Don’t tell him I said that.)

I suspect I’m not alone. Which is why I’m sheepish about reminding employers that the Connecticut Department of Labor requires that employers with 75 or more employees as of a date certain, must file a report with the agency.

The Department has posted a link to it on their Wage & Workplace Standards page with a "New" next to it.

But it appears that someone forgot to get the memo to update the form with new dates and the so-called "new" form hasn’t been updated since last year.  (UPDATE: The form now appears to have been updated.)

For example, it says that the deadline for completing the form for calendar year 2010 is April 15, 2010 and suggests looking back 3 years to determine how many employees you have.

Obviously, mistakes happen but hopefully, the Department can quickly remedy this so that employer aren’t confused by it and submit wrong information.

Until the CTDOL fixes it, you may want to refrain from filling it out. Just make sure to check back in a few weeks so that information CAN be filled out properly.
